Meets physiologic data transmission for multiple codes within the RPM set:
Initial education and secure link delivery qualifies under CPT 99453. No hardware provisioning.
Physiologic data captured and transmitted automatically meeting 99445 and 99454 requirements for practice expense costs for services within a 30-day period.
Collection and interpretation of physiological data requires a minimum of 30 minutes of time, each 30 days, meeting 99091 and does not require interactive communication.
Live, interactive communication of the care team with the patient/caregiver is required for 99457, 99458, and 99470 with varying time allocations.
